Online Clinical History

Worcester Surgical Center offers patients the convenience and privacy of a secure, on line registration process. All patients are directed to please go online today to complete your registration using the login information below. You will be asked about your health history, medications, and previous surgeries. **It is a mandatory requirement that all patients complete a Pre-op medical health history questionnaire online health history assessment**

If a patient does not have access to a computer or is unable to complete on their own, they should be directed to contact Worcester Surgical Center (508-754-0700) and a Pre-op Screening Nurse will assist the patient with completing this over the phone.

New Patients:

Once your procedure/surgery has been successfully scheduled here at Worcester Surgical Center, you will receive your own unique link. Your link to the online health assessment will be sent to either your email or cell phone. Please verify with the surgeon's office that your cell phone number and email are correct.

You must "Opt In" to the text messages to receive the link to get into the questionnaire.

Once your Pre-op medical health history questionnaire is completed, click save.

**Failure to complete your Pre-op medical health history questionnaire will result in postponement of your surgery/procedure. ** Once completed, a nurse will contact you to review your information with you.

Returning patients:

Once you are in our new system, you will not be required to complete another questionnaire. A Pre-op Screening Nurse will contact you and update any changes to your medical history prior to surgery.
